ABSTRACT

An increasing number of complex interventions are being carried out as minimally invasive procedures. More complex endovascular procedures, means longer procedure times. During this time, patient and staff are exposed to X-ray and patients get injected with contrast agent. All this is adding cost pressure to the hospitals and the payer.
X-ray is considered the gold standard for imaging. However, 2D navigation is still challenging for complex procedures for which there is a growing need to see devices in 3D and in relation to the anatomy. At the same time, physicians want to reduce exposure to radiation for themselves and their patients.
Philips LumiGuide is a breakthrough device guidance solution for minimally invasive surgery exclusively integrated with Philips angiography systems. The platform lets physicians visualize their wires and catheters inside the patient vasculature in real-time and in 3Dusing light instead of X-ray.
LumiGuide is powered by Fiber Optic RealShape (FORS) technology. This technology uses light travelling through a hair-thin optical fiber which is integrated within the guidewire. By pulsing light into the fiber and analyzing how it is reflected, the full shape of devices can be reconstructed and visualized. The whole fiber is visible in real time and thereby the guidewire. Standard catheters can be utilized to slid over the guidewire. The software suite combines the 3D spaces of patient anatomy, X-ray imagery and LumiGuide shape and lets physicians see full shape of the devices in 3D, in real time, in distinctive colors and from any viewing angle.
By using light instead of X-ray for device navigation, considerably less radiation exposure for patients, operators and supporting staff is reported.
LumiGuide solution is indicated for aortic, vascular and peripheral vascular procedures. 14 peer-reviewed articles have been published about FORS technology. Clinical studies in complex aortic procedures have shown a 37% reduction in procedure time [1] and a 70% Reduction in Fluoroscopy rate during navigation [2].
To date 14 hospitals are equipped with LumiGuide worldwide and around 900 procedures have been performed with this technology. We believe that LumiGuide has the potential to not only improve the lives of patients, but also the lives of our clinical users, by significantly reducing their exposure to X-ray.
